The Horizon ProResp partnership is a joint venture between Huron Perth Healthcare Alliance and ProResp, Ontario’s largest community respiratory therapy provider.

This partnership ensure that before you leave the hospital your home care respiratory therapists are working in collaboration with your hospital’s team to get you home and help keep you there.

  • In May, 2006, Lesley McDonald was 26 weeks pregnant with twins. That’s when she was diagnosed with twin transfusion syndrome and then HELLP, a dire, life threatening situation for mother and children. It was time to deliver the twins by c-section—14 weeks early.

    Tyler (the recipient twin) was immediately rushed out the door and taken to the neonatal intensive-care unit. Brandon (the donor) was taken to a room directly off the operating room for immediate attention. Both had under developed lungs, were unable to breathe on their own and were put on ventilators and IVs.

    Incredibly sick and recovering from the procedure, it would be three days until Lesley saw her two boys. When she finally saw them, Brandon was on an oscillator ventilator.

    “His lungs were so under developed, we couldn’t even hold him for 60 days,” Lesley recalled.

    Lesley was released from the hospital five days later, but wasn’t able to take Brandon and Tyler home; they had to be kept on ventilators and monitored until their lungs developed. Eventually, after three months, Tyler could come home and after seven months, Lesley had both her boys at home.

    But that part was scary, too—until the McDonald’s met ProResp. Thanks a joint venture between Huron Perth Healthcare Alliance and ProResp, the McDonald’s were introduced to ProResp in hospital, giving them piece of mind that the transition from hospital to home would be seamless. 

    To go home both Brandon and Tyler required oxygen, which was set up by ProResp. “The day Tyler was discharged Sandra Mohr from ProResp met us at the house, explained all the equipment and how it worked and was always on call to help.”

    While ecstatic to finally have her two boys at home, Lesley admits it was difficult. “At the hospital, you had the nurses take care of them. Now you have two kids at home hooked up to chords, two kids on feeding tubes, there was equipment all over the house – it was chaotic,” said Lesley.

    ProResp has been at the McDonald’s side ever since—helping them give their boys the best life possible and make memories together as a family.

    “It doesn’t stop us from doing the things we want to do. All families go through really stressful times and we choose to be laidback, positive and happy. It’s worked well for us,” said Lesley.

This integrated care model allows Huron Perth Healthcare Alliance’s frontline caregivers and management to work directly with ProResp’s frontline caregivers and management to ensure you get the best care possible.

Our teams regularly measure patient satisfaction and performance and report this information back to senior management of the hospital and ProResp.
